Health care professionals

Professionals and practitioners including Health Visitors, Midwives, GPs, Children’s Centres, Nurseries, Schools, Social Prescribers, Voluntary organisation staff, Social Workers, can refer a family to our services.

Please use this form if you would like to refer a family for 1:1 home-visiting or group support. If you are making a referral for a family, you must have their consent. We support families with at least one child under the age of 5 years (including first-time mothers in their third trimester) who live in the borough of Wandsworth.

HOME-START LONDON

Home-Start Wandsworth is a member of the Home-Start London consortium, which is made up of 13 Home-Start schemes reaching as far as Barnet in the north to Sutton in the south and Redbridge in the east to Hillingdon in the west. Together the consortium members create a larger footprint than they would individually to have a greater impact across London.
